OUR SERVICES

Changing Lives Networks, Inc. will coordinate with other service providers to support a whole health and wellness approach to recovery.

Peer Support Specialists (16-21) will help you identify and connect with the support you need to achieve your goals independently. 

 Transitional Youth Services (18 & up) refers to structured, community‑based support programs that help young people (typically ages 16–21) with mental health or substance use challenges develop the skills, stability, and resources needed to live independently

In Home Therapy Services (3 -20) we provide evidence-based therapy and care coordination in the home for children and teens with complex needs and family challenges.

 Counselors and Social Workers may contact Changing Lives Networks, Inc to request support services if they are serving a consumer with serious mental illness living in Surrounding Counties.

PEER SUPPORT SERVICES

 

  • Provide guidance and support for individuals in recovery from serious mental illness, trauma, and addiction from Peers with lived experience.

  • Link to community resources and promote self-advocacy and self-directed recovery

  • Provide ongoing support through recovery transitions such as housing, vocational rehabilitation, recovery from serious illness or injury, and aging out of support systems

  • Assist individuals with identifying common stressors in order to develop coping strategies to reduce symptoms

  • Assist with building a support network to create a fully integrated sustainable recovery system
